You MUST HAVE a doctor’s order/prescription and completed assessment to be tested for COVID-19. CALL your primary care provider BEFORE YOU GO!
Local Agencies Offering Modified Services:
The Independence Center: Beginning Monday, March 16, 2020, our buildings (located at 729 and 711 S. Tejon) will be closed to visitors, including consumers, clients, and vendors until further notice. Our staff will still be available to assist you by phone (719-471-8181) and email (firstname.lastname@example.org) during our regular business hours of 8 a.m. – 4:30 p.m. Monday through Friday.
Community Partnership: Community Partnership Family Resource Center staff will continue to be available to our community by phone, and appointments as appropriate, throughout community closures. We are working very closely with other service providers in our community to connect families as quickly as possible to available resources. The best way to refer families during this time is to provide the office phone number for Community Partnership, 719-686-0705. Phones will be manned Monday – Friday 8-5 (remotely if necessary) and we will be checking voicemail and returning calls over the weekends. Families who leave a voicemail will receive a return call from one of our family advocates within 24 hours.
